When levels are already adequate, the picture changes sharply. A 2021 systematic review and meta-analysis in Nutrients pooled 16 randomized trials covering 6,276 participants and found no effect of B12 supplementation on cognitive function or depressive symptoms in people without an overt shortfall; only one trial examined idiopathic fatigue, too few to pool. The NIH Office of Dietary Supplements states plainly that B12 supplementation does not appear to improve athletic performance or endurance in people whose B12 status is sufficient. Marketing language around B-complex energy shots runs well ahead of that evidence. For vitamin D, a 2017 PLoS One pharmacokinetic study found a single 100,000 IU intramuscular dose raised 25-hydroxyvitamin D comparably to daily oral supplementation at 28 days, with a slower decline afterward, which is a convenience and adherence argument rather than a superiority one; the small sample (18 people in the injection cohort) limits how far it generalizes. Glutathione is the weakest link in the category. No injectable glutathione product is FDA-approved, and a widely cited 2016 review in the Indian Journal of Dermatology, Venereology and Leprology concluded that the skin-lightening reputation of intravenous glutathione is not supported by trial evidence and has drawn regulatory warnings abroad. Contamination is a documented, separate risk: a 2018 report in Epidemiology and Infection described seven patients who developed fever, vomiting, and low blood pressure within two hours of receiving compounded glutathione infusions traced to endotoxin-contaminated imported powder. Compounded injectables do not undergo the premarket review that approved drugs do, so product provenance, sterile technique, and who is administering the shot carry as much weight as the ingredient itself.
