A randomized, double-blind, placebo-controlled trial found that drinking hydrogen-rich water around exercise lowered self-rated fatigue in untrained adults and improved endurance and exertion ratings in trained adults. The effect was measured around a bout of exercise in healthy people, not for standalone long-term fatigue.

How strong the finding is
One randomized, double-blind, placebo-controlled trial (Mikami 2019, PMID 31251888). In the untrained experiment, psychometric fatigue on visual analogue scales fell significantly in the hydrogen group after mild exercise. In the trained experiment, endurance and fatigue improved as judged by maximal oxygen consumption and Borg's scale. The published abstract reports significance but no p-values.
What complicates this
This measured acute, exercise-related fatigue in healthy volunteers, which is a different thing from persistent daily fatigue. The 2024 systematic review that includes exercise-capacity outcomes judged the hydrogen-water literature as a whole to be preliminary and dependent on small samples, so a single positive trial should not be read as a settled finding.

Molecular hydrogen shows antioxidant and anti-inflammatory activity relevant to immune signaling, and a 2015 review reports improvements in clinical end-points and surrogate markers across several human trials spanning metabolic disease, chronic systemic inflammatory disorders, and cancer. That same review notes that dosage, administration, adverse reactions, and use in specific populations remain poorly characterized, and the immune-specific work cited alongside it is a rat cardiac-transplant model. This is an early mechanism-level signal rather than a confirmed immune benefit.

The same review notes the underlying mechanisms are not yet fully explained.

A 30-minute bath in supersaturated hydrogen-rich water taken immediately after a muscle-damaging workout was followed by lower perceived soreness and a smaller rise in creatine kinase than an identical bath in plain water, in a double-blind crossover trial of six young men. The authors note this was a single bathing session with no follow-up beyond 24 hours.

In a 24-week randomized, double-blind, placebo-controlled trial in 60 adults with metabolic syndrome, high-concentration hydrogen-rich water improved biomarkers of inflammation and redox homeostasis alongside lower cholesterol, glucose and HbA1c (P<0.05). It is the longest human trial identified here and remains a single unreplicated study.

What complicates this
Single trial (n=60), not independently replicated, and the abstract does not name which specific inflammatory markers (for example hs-CRP versus IL-6) moved or report per-marker effect sizes, so the result should not be generalized beyond metabolic-syndrome-adjacent populations.

In a pilot single-blind randomized placebo-controlled trial of 32 adults with Long COVID, 14 days of hydrogen-rich water lowered Fatigue Severity Scale scores compared with placebo water (p = 0.046, d = 0.696) and improved walk-test distance, chair-stand performance, and sleep-quality scores, while showing no significant difference in breathlessness or in depression, anxiety, and stress scores.

Findings are gene-expression and cellular surrogate markers in healthy adults (n=38), not clinical inflammatory-disease outcomes, and have not been replicated in a population selected for chronic systemic inflammation.

How strong the finding is
One pilot single-blind trial (Tan 2024, PMID 38794767, n=32). Against placebo water, the hydrogen group showed lower Fatigue Severity Scale scores (p=0.046, d=0.696), a longer six-minute walk distance (p<0.001, d=1.010), better 30-second chair stand results (p=0.002, d=1.190) and better Pittsburgh Sleep Quality Index scores (p=0.012, d=1.274). The mMRC dyspnea scale showed no difference (p=0.556) and the DASS-21 mood measure showed none either (p>0.143).
What complicates this
Single-blind design, 16 people per arm, and a fatigue p-value of 0.046 sitting just under the significance threshold all limit confidence. Two of the measured outcomes, breathlessness and mood, showed no benefit. Long COVID fatigue is also not the same entity as primary chronic fatigue syndrome or ME/CFS.

In an 8-week randomized, double-blind, placebo-controlled crossover trial in 36 people with type 2 diabetes or impaired glucose tolerance, 900 mL per day of hydrogen-rich water was associated with lower modified LDL cholesterol (about 15 percent), lower small dense LDL and a lower urinary oxidative-stress marker, with oxidized LDL showing only a non-significant trend. Glucose tolerance testing normalized in 4 of the 6 participants who had impaired glucose tolerance, an early signal in a very small subgroup rather than a broad efficacy finding.

In a study published in January 2026, hydrogen eased psoriasis-like skin inflammation in mice and in cultured cells by suppressing a specific immune-signaling pathway (cGAS-STING). This is an early mechanistic clue about hydrogen and autoimmune-type inflammation in a laboratory model, not evidence that it changes immune function in people.
